Evidence that must be submitted on the day of application
1. A copy of the student’s house registration – father – mother and evidence of name-surname change (in case of change) amount 1 set.
2. A copy of the identity card of the father-mother and parents, 1 set.
3. 1 copy of birth certificate.
4. Two 1-inch photos of the front face.

6. Academic results record (Por Por. 1).
7. Desirable character development form (Por Por. 4).
8. Individual learner quality development report form (Por Por. 6).
9. Cumulative record documents (Por Por. 8).
10.Learning results notebook (Por Por Por 9).
